APV/Spectrum OPT OUT Criteria

On this Page you will find the Criteria and Documentation needed for Opting Out of the APV/Spectrum Bulk Service Plan. Please read carefully the criteria before sending application.

Send you application to apv.spectrum@fsresidential.com with the words "application attached"  in the subject line or bring  to the Mary Jane Arrington Gymnasium. to submit. 

If you can answer yes to 1 or more of these questions you may be eligible to opt out of the APV/Spectrum Bulk Service Plan. These are government established opt out options.

  • Is the property owner legally blind or hearing impaired and all other residents of the home are also legally blind or hearing impaired?

  • Does the property owner receive Supplemental Security Income?

    • This is NOT Social Security Retirement Income. It is a special payment made to select persons

  • Does the property owner receive food assistance from the Florida Department of Children and Family Services?

If you  answered yes to 1 or more of these questions Please Proceed

HOW TO APPLY 

  1. DOWNLOAD AND FILL OUT THE APPLICATION COMPLETELY AND ADD DOCUMENTS REQUESTED

    1. AN INCOMPLETE APPLICATION AND DOCUMENTATION WILL NOT BE PROCESSED

  2. EMAIL YOUR APPLICATION AND DOCUMENTATION TO APV.SPECTRUM@FSRESIDENTIAL.COM 

    1. OR DIANE MCGOUGH AT 863-427-0900 ext. 709 TO MAKE AN APPOINTMENT TO BRING IN YOUR DOCUMENTATION
